Let us know your home wants to join SPA-LTC by sending us an Email to  spa-ltc@unb.ca 

Within a few days, we'll send your home a Welcome Package including : the Information Sheet, the Consent Form, and provide you with an overview of what SPA-LTC is about and how to participate.

Read the SPA-LTC Information Sheet included in your Welcome Package. The Information Sheet explains what your home is agreeing to.

When you're ready and reviewed it complete the Consent Form and send it back to spa-ltc@unb.ca. 

Choose the staff members who will lead SPA-LTC in your home. This is your Champion Team. Send us their names and email addresses so we can get them set up.
